We seek applicants for the role of MDS Coordinator. In this role you will be responsible for the timely scheduling, completion, and transmission of MDS. You will monitor and guide the completion of PPS and OBRA assessments and RAPS. You will oversee the gathering of info for assessments and care plans. You will assess prospective residents in conjunction with other teammates. You will schedules and facilitates care plan conferences. You will input or supervise the input of MDS assessments into the RCS, the transmission to the state, and obtain receipt of validation. You will perform other tasks as assigned.RequirementsCandidates must be a graduate of an licensed institution of nursing and be licensed as an RN in the state of Pennsylvania. Must enjoy working with the elderly and their families, and must have MDS/Care Plan background with good technical skills. Our perfect candidate is superior in CMI and MDS 3.0.
